장폐색(ileus)의 발생 기전

장의 수축 운동

생리적인 bowel motility는 다양한 neural network와 neurohormonal mediator의 상호작용으로 발생하는 복잡한 과정이라 할 수 있다. 금식 상태에서는 GI tract의 조화된 수축이 일어나는데, 이를 migrating motor complexes (MMC)라 한다. 이 수축 과정은 다음과 같은 3 phase로 나타난다:

- Resting phases

- Moderate amplitude의 intermittent contractions

- High-pressure waves

 

정상적인 장운동 기전

음식물이 장 안으로 들어가게 되면 MMC는 종료되는데, 소화된 음식물이나 chyme은 intestinal wall의 smooth muscle layer의 조화된 수축 (peristalsis)에 의해 GI tract를 내려가게 된다 (propelled through). 이러한 과정은 myenteric nerve plexus, submucosal sensory nerve plexus, submucosal motor nerve plexus, interstitial cells of Cajal)로 구성된 enteric nervous system (ENS)에 의해 조절된다. 이 ENS는 intestinal wall로부터 sensory information을 받아서 vagus nerve, splanchnic nerve, pelvic nerve 등의 visceral sensory afferents network를 통해 central nervous system로 전달한다. 또한 ENS는 이러한 nerve로 구성된 visceral motor efferents와 intestinal smooth muscle을 연결한다.

 

ENS와 intestinal smooth muscle activity는 sympathetic singaling에 의해 억제되는 반면, parasympathetic cholinergic signaling에 의해 촉진된다.

 

또한 ENS는 CNS의 조절과 독립적으로 기능할 수 있다. 이는 substance P, vasoactive intestinal peptide, nitric oxide 등의 mediator를 통한 autonomic nervous system을 통해 이루어진다.

 

 

Ileus의 원인

Ileus는 생리적인 neural signaling이나 neurohormonal networks가 제대로 기능하지 않을 때 발생할 수 있으며 그 원인은 다음과 같다:
증가된 sympathetic activity에 의한 억제성 neuroenteric signaling
bowel wall이나 그 주변 구조물의 inflammation
억제성 gastrointestinal peptide나 endogenous opioid의 paracrine, endocrine activity
진통 목적의 exogenous opioids 투여

수술 후 발생하는 ileus

복부 수술 후에 ileus가 흔하게 올 수 있다. 보통 small bowel motility는 수술 후 24시간 이내에 돌아오고, gastric motility는 24에서 48시간 이내에, colonic motility는 48에서 72시간 이내에 돌아온다고 알려져 있다.

하지만 최근의 연구들은 postoperative intestinal dysmotility기간이 좀 더 짧다고 보고하고 있다; gastric motility와 small intestinal motility는 수술 후 수 시간 이내에 돌아오고, colonic motility는 수술 후 2, 3일 이내에 돌아온다.

만약 수술 후 bowel motility가 돌아오는 기간이 일반적인 예상 기간을 넘어가는 경우(개복 수술은 5일, 복강경 수술은 3일),
pathologic postoperative ileus를 의심해야 하며 그 원인을 찾아야 한다.
